How to perform:

Open an existing form or create a new form – here.

Add a category if needed – here.

Click Add Question Group.

Title – Enter a title for the question group.

Type – Choose Default or Sub-Action.
Only when selecting Sub-Action can you add entity-type questions such as Locations, Employees, etc.

Required – Mark if all questions in the group must be answered.

Active – Define whether the group is active in the form.

Print Text – Text that will appear in the print output.

Hide Title – Option to hide the group name in the interface.

Print? – Define whether to include the group in printed outputs.

Auto Create Sub-Action for Each Group – Checking this box will allow creating a task rule for responses to this question group (Guide – here).
After saving, the question group will appear like any other question in the form. Click it to open.
Now you can add multiple questions within the group. Each added question will allow repeated answers for that same question.
